Self referral, healthcare and GP referrals are accepted.
All referrals are to be faxed to (03) 5332 8939, or sent to relevant location;

Faxed referrals will be given a higher priority over mailed referrals!!!!!
Appointments
Once the referral is received, Sheree will contact you with a suitable time and date for your appointment. Initial individual appointments are 30-40 minutes and Review appointments are 15-20 minutes. The amount of visits will depend on the needs of the client or request from your healthcare professional. Longer consultations can be negotiated.
What to Bring with You
Medicare, Concession, Pension, Private Health Insurance or Department of Veteran Affairs (DVA) cards |

Referral letter and EPC plan (if eligible) from your GP or referral from your Endocrinologist or other healthcare professional. (Unless you are Self Referring) |

| Copies of recent blood tests i.e.: Haemoglobin (HbA1c), Oral Glucose Tolerance Test (OGTT), fasting blood glucose, urine tests, cholesterol, blood pressure, pregnancy, and any other relevant test results. |
If all the relevant information is not made available at the time of your appointment this may cause a delay in your treatment.
Fees
Enhanced Primary Care Plans allocated visits will be bulk billed. If Medicare Australia rejects the claim, an invoice fee of $55.00 will be required to be paid in full by you.
Department of Veterans Affairs Clients will be bulk billed if a current D904 referral is provided by your GP for diabetes services.
Private and Pregnancy Consultations are to be discussed with Sheree.

To cancel an Appointment:
Telephone the office during business hours 9am-5pm.
Please cancel the appointment at least 1 day prior so that your appointment time can be allocated to another patient who is seeking treatment.
Note: Continuous cancellations will incur a fee which will be invoiced to the client.
